Chart notes are audited utilizing guidelines for proper documentation and CMS 1995 or CMS 1997 guidelines for E&M coding.

The price of audits will vary depending upon the size of the facility being audited, the place of the audit and the turnaround time needed by the client. Upon completion, the auditor presents a report outlining documentation deficiencies and recommendations for improvement.
